RENAL CARE COORDINATOR

The Renal Care Coordinator (RCC), Jennifer Woods, is employed by Fresenius Medical Care (FMC), a dialysis provider, as part of a collaborative agreement with Valley Nephrology Associates. The goal of the RCC is to help assure that all patients with Late Stage Chronic Kidney Disease (LSCKD) are informed of their options and make the best individual decision regarding their options, should their disease progress, requiring dialysis.

The RCC will help you make the following decisions regarding your Kidney Disease:

  • RCC is available to meet with you in person or over the phone to discuss concerns and answer questions
  • Explain lab values and trends, and what they represent to the patient with LSCKD
  • Dialysis and transplant options will be briefly discussed by RCC and patient will be scheduled for a TOPS class for full discussion of options
  • The RCC assures that each patient has their dialysis access placed in a timely manner and will monitor access for maturation and intervention, if needed

PROVIDING THE MOST COMPREHENSIVE KIDNEY CARE IN SOUTHWEST VIRGINIA

NOCTURNAL HEMODIALYSIS

Nocturnal Dialysis takes place while you sleep. This is a longer and slower dialysis treatment that takes place over 6-8 hours, 3 nights per week. Nocturnal dialysis can be done at your home or in-center and can have many benefits to your life style and health. Your provider will help decide if this would be an appropriate treatment option for you.

PEDIATRIC

Pediatric nephrologists specialize in the diagnosis and management of many kidney related conditions and disorders, including: high blood pressure, kidney stones, electrolyte disorders, urinary tract infections, urinary reflux, problems with blood or protein in the urine, and kidney failure. We also help children with diabetes take care of their kidneys. Pediatric nephrologists specialize in the care of patients from newborns to young adults.

TRANSPLANT

VNA and the University of Virginia developed an outpatient nephrology program at VNA, reducing the amount of travel time to and from Charlottesville for these patients. A transplant coordinator located at VNA provides coordination between the patients, the University of Virginia and VNA to provide pre- and post-transplant services locally.

HOME HEMODIALYSIS

Home Hemodialysis is dialysis done in your home. This is done on the same schedule 3 days a week, 3-4 hours per treatment but offers a lot of flexibility. The flexibility gives you more independence and can have many benefits for your life style and health.

Nephrology is a specialty that focuses on the diagnosis and treatment of diseases and disorders of the kidneys. The kidneys maintain normal fluid and electrolyte balance in the body that are vital to live a normal, healthy life. As nephrologists, we diagnose and put together the best treatment plan for our patients to maintain a healthy life.
